Mitchell Henderson (in the icon) was a young kid who killed himself, people checked his myspace and one of the last posts he'd made was about losing his ipod, so it became a huge joke that he'd offed himself because of that.
I think what happened next was his friends either set up a memorial myspace/ webpage for him, or comments were left on his original myspace as commiseration from school friends of his. Anonymous found out about this, and started trolling them with comments about ipods, etc. One of the girls who went to school with him left a lengthy comment about how they were all cruel heartless monsters, and said "he was an hero." The friends and family reacted badly, anonymous took it further, it grew and eventually became a meme.
A fair while ago it was proper grammatical practice to write 'a' as 'an' before a word that started with an 'h', or any word starting with a vowel sound. It's not done nowadays, 'an' generally only being used before a word starting with a vowel (an octopus, a squid, etc) although some people still do it. It looked very out of place, and anonymous went to town with it, using 'an hero' as a paraphrasing of 'kill yourself.' It can be used differently, 'an hero' is someone who's killed themself, 'become an hero' means 'go kill yourself,' etc.
